Website Video Shoot Info Form

Please see the form below to answer some questions so we can properly plan out everything before the shoot. This will also help with post production and credits.

This video will be posted on the main landing page of the website. We also will have a photographer on board for shoots if you would like! We are also planning to put this on YouTube, but you can opt out of being credited in the description if it is uploaded there.

Time: 3:00pm. We will start at 3:30 so this gives time for walkthrough
